other پزشک خانواده؛ آزمون بزرگ حکمرانی سلامت در ایران Family Physician: The Great Test of Health Governance in Iran قلب و عروق قلب و عروق پژوهشي اصیل Original Research <div style="text-align: justify;">برنامه پزشک خانواده، نزدیک به دو دهه است که به عنوان راهبردی کلیدی برای ساماندهی نظام سلامت ایران مطرح شده، اما همچنان در مرحله وعده باقی مانده است. اجرای این برنامه می تواند نظام سلامت را از رویکرد واکنشی و درمان محور به سمت مراقبت پیوسته و پیشگیرانه سوق دهد، هزینه ها را کاهش دهد و عدالت را در دسترسی به خدمات ارتقا بخشد. تأخیر طولانی مدت در اجرا، بیش از آنکه ناشی از کمبود منابع باشد، ریشه در چالشهای حکمرانی، نبود هماهنگی بین بخشی، ضعف زیرساختها و فقدان عزم راهبردی دارد. تحقق این برنامه، نیازمند سیاستگذاری یکپارچه، تأمین منابع پایدار، سرمایه گذاری در نیروی انسانی و فرهنگسازی عمومی است و آزمونی جدی برای کارآمدی نظام مدیریت سلامت کشور محسوب میشود.</div> For nearly two decades, the family physician program has been proposed as one of the most transformative initiatives for Iran's health system. Conceived as the cornerstone of an efficient, equitable, and preventive healthcare model, this program promises to replace fragmented, reactive care with a continuous, coordinated, and patient-centered approach. It aims to strengthen primary healthcare, streamline the referral system, reduce unnecessary costs, and ultimately restore dignity and clarity to patients navigating a complex medical landscape. Despite its undeniable advantages and repeated emphasis in national development plans, the program remains largely unimplemented. This persistent delay reflects deeper governance challenges rather than technical or financial barriers alone. The absence of a unified, cross-sectoral commitment has fragmented responsibilities among various institutions, while policy incoherence and short-term decision-making have undermined long-term planning. Furthermore, inadequate investment in infrastructure development, particularly the establishment of an integrated electronic health record system, has hindered the seamless coordination required for effective implementation of this program. The success of the family physician program hinges on a fundamental shift in health governance. This requires a clear strategic vision, sustainable resource allocation, transparent accountability mechanisms, and active engagement of all stakeholders, including healthcare providers, insurers, and the public. Equally important is investment in human capital, empowering general practitioners through targeted training and creating meaningful career pathways that enhance their motivation and professional standing. Public awareness and cultural acceptance are also essential to transform the community's understanding of health from a curative to a preventive paradigm. The family physician program is not merely a policy option; it is a critical test of Iran's capacity for strategic governance in health.
