fa کاهش سطح هوشیاری به دنبال مصرف قرص باکلوفن: یک گزارش موردی فارماکولوژی فارماکولوژی گزارش موردي Case report <table align="center" border="0" cellpadding="0" cellspacing="0" dir="rtl" style="width:604px;border-collapse:collapse;" width="604"> <tbody> <tr style="page-break-inside:avoid;height:1px;"> <td style="width: 604px; padding: 0cm 5.4pt; height: 1px; vertical-align: top; text-align: justify;"><span style="font-family:Tahoma;"><span style="font-size:11.0pt;"><strong>مقدمه:</strong> </span><span style="font-size:10.0pt;">باکلوفن یک آگونیست&nbsp; فعال کننده&shy; ی مرکزی گیرنده&shy; ی گاماآمینو بوتیریک اسید است و در درمان اسپاسم&shy; های عضلانی به طور وسیعی مورد استفاده قرار می&shy; گیرد. دوز خوراکی باکلوفن در گرفتگی &shy;ها 40 تا 80 میلی گرم روزانه است. مسمومیت با این دارو در دوزهای بالا رخ می&shy; دهد ولی در بیمار حاضر مسمومیت با 75 میلی&shy;گرم رخ داده است</span>.</span><span style="font-family:B Nazanin;"></span><br> &nbsp;</td> </tr> <tr style="page-break-inside:avoid;height:1px;"> <td style="width: 604px; padding: 0cm 5.4pt; height: 1px; vertical-align: top; text-align: justify;"><span style="font-family:Tahoma;"><strong><span style="font-size:11.0pt;">گزارش مورد:</span></strong> <span style="font-size:10.0pt;">خانمی 76 ساله که با کاهش سطح هوشیاری ناگهانی مراجعه نموده است. سابقه ی بیماری&shy; های دیابت، سکته مغزی، فشار خون و مشکلات کلیوی را ذکر نمی کند. ایشان به علت اسپاسم عضلات گردن و شانه قرص باکلوفن 25 میلی&shy;گرمی خوراکی مصرف نموده و بعد از مصرف 75 میلی&shy;گرم، دچار علائم مسمومیت شده است. سایر علل احتمالی کاهش سطح هوشیاری از قبیل سکته مغزی، مصرف تریاک و سابقه دیابت رد شد و تشخیص مسمومیت با باکلوفن بررسی شد.</span><br> <span dir="RTL"><span style="font-size:11.0pt;"><strong>بحث و نتیجه گیری:</strong> </span><span style="font-size:10.0pt;">توصیه می شود که تجویز باکلوفن در افراد مسن و بیماران با اختلال کلیوی با احتیاط صورت گیرد و به بیمار یا همراهان در زمینه&shy; ی شناخت عوارض و علائم مسمومیت باکلوفن و مراجعه&shy; ی سریع به بیمارستان آموزش داده شود. همچنین توصیه می&shy; شود در صورت شواهدی از عدم کارکرد صحیح کلیه، از داروهای جایگزین باکلوفن برای کنترل درد و اسپاسم عضلانی استفاده گردد</span></span></span><span style="font-family:B Nazanin;"></span></td> </tr> <tr style="page-break-inside:avoid;height:1px;"> <td style="width: 604px; padding: 0cm 5.4pt; height: 1px; vertical-align: top; text-align: justify;"></td> </tr> </tbody> </table> <div style="text-align: justify;"></div> <div style="text-align: justify;"><strong><em>Background:</em></strong> As a central activator agonist of the gamma-amino-butyric acid receptor, baclofen is widely used in the treatment of muscle spasms. The oral dose of baclofen in cramps is 40 to 80 mg daily. Poisoning with this drug occurs at high doses, but it occurred with 75 mg in the patient who participated in the present study.<span dir="RTL"></span><br> <strong><em>Case Report</em></strong>: A 76-year-old woman with a sudden decrease in the level of consciousness presented herself. She does not mention a history of diabetes, stroke, hypertension and kidney problems. She has been taking oral baclofen tablets due to spasm of the neck and shoulder muscles, and has been poisoned after taking 75 mg.<br> <strong><em>Conclusion:</em></strong> We recommend that baclofen be administered cautiously in the elderly and in patients with renal impairment. Moreover, the patient or the companion should be informed not only about the signs and complications of the consumption of baclofen but also about the necessity of referring to the hospital promptly.
