fa الگوی مقاومت آنتی‌بیوتیکی ایزوله‌های کلبسیلا پنومونیه جدا شده از بیماران بستری در بخش مراقبت‌های ویژه بیمارستان شهید رحیمی خرم‌آباد در سال ۱۴۰۱ Antibiotic Resistance Pattern of Klebsiella pneumoniae Strains Isolated from Patients Admitted to the Intensive Care Unit of Shahid Rahimi Hospital in Khorramabad in 2022 میکروب شناسی میکروب شناسی پژوهشي اصیل Original Research <table align="right" class="Table" dir="rtl" style="width:596px; border-collapse:collapse; margin-left:9px; margin-right:9px" width="596"> <tbody> <tr> <td style="width: 596px; padding: 0in 7px; height: 1px; text-align: justify;" valign="top">مقدمه: کلبسیلا پنومونیه یکی از انواع باکتری‌های بیماری‌زای انسانی مهم است که پیدایش مقاومت آنتی‌بیوتیکی مشکلات درمانی زیادی را برای حذف آن ایجاد کرده است؛ لذا مطالعه حاضر باهدف بررسی الگوی مقاومت آنتی‌بیوتیکی باکتری کلبسیلا پنومونیه در بیماران بستری در بخش مراقبت‌های ویژه بیمارستان شهید رحیمی خرم آباد در سال ۱۴۰۱ انجام شد. مواد و روش ها: این مطالعه توصیفی-تحلیلی بر روی ۱۲۰ نفر از بیماران بستری در بخش مراقبت‌های ویژه بیمارستان شهید رحیمی خرم آباد در سال ۱۴۰۱ انجام شد. پس از جمع‌آوری ایزوله‌های کلبسیلا پنومونیه، الگوی مقاومت آنتی‌بیوتیکی ایزوله‌ها ارزیابی شد. سپس ارتباط میان مقاومت آنتی‌بیوتیکی با متغیرها با نرم‌افزار  SPSS  نسخه ۲۵ تجزیه‌وتحلیل شد. یافته ها: میانگین سنی بیماران ۴۴/۲۶ ± ۰۹/۵۵ سال بود. بیشترین سویه کلبسیلا پنومونیه از نمونه ادراری (۳/۴۸ %) به دست آمد. بیشترین حساسیت آنتی‌بیوتیکی نسبت به آمیکاسین (۸/۶۵ %)، جنتامایسین (۸/۵۰ %) و سیپروفلوکساسین (۲/۴۹ %) و بیشترین مقاومت نسبت به سفکسیم (۳/۶۸ %)، سفوتاکسیم (۷/۶۶ %) و سفتریاکسون (۸/۶۰ %) گزارش شد. بین بخش مراقبت‌های ویژه بستری (ICU  جراحی، ICU  جنرال، ICU  داخلی، ICU  اورژانس و ICU  اطفال و الگوی مقاومت آنتیبیوتیکی ارتباط معنی‌داری وجود داشت (05/0  P<). بحث و نتیجه گیری: با مشخص نمودن الگوی حساسیت آنتی‌بیوتیکی بخش مراقبت‌های ویژه در منطقه مورد بررسی و گزارش آن به پزشکان می‌توان نتایج را در درمان‌های تجربی مورد توجه قرار داد که خود سبب پاسخ‌دهی بهتر بیماران نسبت به دارو و کاهش مقاومت آنتی‌بیوتیکی در بخش مراقبت‌های ویژه بیمارستان را سبب می‌شود.</td> </tr> </tbody> </table> <div style="text-align: justify;">Background: Klebsiella pneumoniae is one of the essential types of human pathogenic bacteria. The emergence of antibiotic resistance has caused numerous treatment problems for its elimination. The present study aimed to assess the antibiotic resistance pattern of K. pneumoniae bacteria in patients admitted to the intensive care unit (ICU) of Shahid Rahimi Hospital in Khorramabad in 2022. Materials and Methods: This cross-sectional descriptive study was conducted on 120 patients admitted to the ICU of Shahid Rahimi Khoramabad Hospital in 2022. After collecting K. pneumoniae isolates, the antibiotic resistance pattern of all isolates was evaluated. Thereafter, the relationship between antibiotic resistance and different variables was analyzed using SPSS software (version 25). Results: In this study, the mean age of patients participating was 55.09±26.44 years. The most isolated strain of K. pneumoniae was obtained from urine samples (48.3%). The highest rate of antibiotic sensitivity was to amikacin (65.8%), gentamicin (50.8%), and ciprofloxacin (49.2%), while the highest rate of antibiotic resistance pertained to cefixime (68.3%), cefotaxime (66.7%), and ceftriaxone (60.8%). The pattern of antibiotic resistance showed a significant relationship with inpatient ICU, surgical ICU, general ICU, internal ICU, emergency ICU, and pediatric ICU (P < 0.05).
