fa طرح تحول نظام سلامت و نوع زایمان در استان لرستان زنان زنان پژوهشي اصیل Original Research <table align="right" class="Table" dir="rtl" style="width:596px; border-collapse:collapse; margin-left:9px; margin-right:9px" width="596"> <tbody> <tr> <td style="width: 596px; padding: 0in 7px; height: 1px; text-align: justify;" valign="top"><span style="font-family:Tahoma;"><span style="font-size:12pt"><span style="direction:rtl"><span style="unicode-bidi:embed"><strong><span lang="AR-SA"><span style="letter-spacing:-.1pt">مقدمه:</span></span></strong> <span lang="AR-SA" style="font-size:10.0pt"><span style="letter-spacing:-.2pt">در کشور های در حال توسعه، آمار و ارقام سزارین روز به روز در حال پیشرفت است و کشور ما نیز ار این قاعده مستثنی نمی باشد. طرح تحول نظام سلامت از سال ۱۳۹۳ اجرا و یکی از اهداف آن کاهش میزان سزارین به ۳۰-۲۵٪ بود. هدف این مطالعه بررسی و مقایسه میزان زایمان سزارین و زایمان طبیعی قبل و بعد از اجرای طرح تحول نظام سلامت در استان لرستان بود.</span></span></span></span></span><br> <span style="font-size:12pt"><span style="direction:rtl"><span style="unicode-bidi:embed"><strong><span lang="AR-SA"><span style="letter-spacing:-.1pt">مواد و روش&shy; ها:</span></span></strong> <span lang="AR-SA" style="font-size:10.0pt"><span style="letter-spacing:-.2pt">این مطالعه یک مطالعه توصیفی مقطعی بود که در سال 1399 انجام شد. جامعه پژوهش شامل تمام زنانی بود که طی سال های 95 </span></span><span dir="LTR" style="font-size:10.0pt"><span style="letter-spacing:-.2pt">-</span></span><span style="font-size:10.0pt"><span style="letter-spacing:-.2pt"> 90 در یکی از بیمارستان های دولتی&nbsp;استان لرستان که طی این سالها مجری طرح تحول سلامت بودند، زایمان کرده بودند. روش نمونه گیری به صورت سرشماری از طریق آمار معاونت درمان دانشگاه علوم پزشکی لرستان بود. داده ها پس از جمع آوری از طریق آمار توصیفی مورد تجزیه و تحلیل قرار گرفت.</span></span></span></span></span><br> <span style="font-size:12pt"><span style="direction:rtl"><span style="unicode-bidi:embed"><span lang="AR-SA"><span style="letter-spacing:-.2pt"><strong>یافته &shy;ها:</strong> </span></span><span lang="AR-SA" style="font-size:10.0pt"><span style="letter-spacing:-.2pt">نتایج مطالعه نشان داد بر خلاف انتظار، به طور کلی میزان سزارین پس از اجرای طرح تحول نظام سلامت نسبت به قبل از اجرای این طرح به میزان 4/3 % افزایش یافت. لازم به ذکر است در برخی بیمارستان ها از جمله بیمارستان عسلی خرم آباد، تامین اجتماعی خرم آباد و کوثر بروجرد میزان زایمان سزارین پس از اجرای طرح تحول نظام سلامت به ترتیب 6/4 %، 5/6 % و 9/5 % افزایش یافته بود.</span></span></span></span></span><br> <span style="font-size:12pt"><span style="direction:rtl"><span style="unicode-bidi:embed"><strong><span lang="AR-SA"><span style="letter-spacing:-.2pt">بحث و نتیجه&shy; گیری:</span></span></strong> <span lang="AR-SA" style="font-size:10.0pt"><span style="letter-spacing:-.2pt">به نظر می رسد با توجه به میزان بالاتر از استاندارد زایمان سزارین در این استان و کارساز نبودن طرح تحول نظام سلامت در این زمینه، نیاز است با استفاده از برنامه های گسترده در راستای افزایش آگاهی و فرهنگ سازی جامعه تلاش های بیشتری صورت گیرد.</span></span></span></span></span></span></td> </tr> </tbody> </table> <div style="text-align: justify;"><span style="font-size:8.5pt"><span style="line-height:17pt"><span new="" roman="" style="font-family:" times=""><b><i><span style="font-size:13.0pt">Background:</span></i></b> <span style="font-size:12.0pt">The health system transformation plan has been implemented since 2014, and one of its goals is the reduction of cesarean rate to 25%-30%. The present study aimed to assess the rate of cesarean delivery and vaginal delivery before and after the implementation of the health system reform plan in Lorestan, Iran.</span><span style="font-size:12.0pt"></span></span></span></span><br> <span style="font-size:8.5pt"><span style="line-height:17pt"><span new="" roman="" style="font-family:" times=""><b><i><span style="font-size:13.0pt">Materials and Methods:</span></i></b> <span style="font-size:12.0pt">This descriptive study was conducted in 2020. The research population included all the women who had given birth in one of the public hospitals of Lorestan province during 2011-2016. The sampling method was the census, which was based on the statistics of the vice-chancellor of treatment at Lorestan University of Medical Sciences. After data collection, they were analyzed using descriptive statistics.</span><span style="font-size:12.0pt"></span></span></span></span><br> <span style="font-size:8.5pt"><span style="line-height:17pt"><span new="" roman="" style="font-family:" times=""><b><i><span style="font-size:13.0pt">Results:</span></i></b> <span style="font-size:12.0pt">The results of the study demonstrated that contrary to expectations, the cesarean section overall rate increased by about 3.4% after the implementation of the health system transformation plan, compared to before the implementation of this plan.
