Since‌ 2014, with the implementation of the‌ &ldquo;Health Transformation Plan,&rdquo; Iran&rsquo;s health system has undergone significant reforms. One of the less explored aspects of this plan is its ethical challenges. The four core ethical principles&mdash;beneficence, non-maleficence, autonomy, and justice constitute the foundation of health policies. Therefore, the present study was conducted as a narrative review to examine the existing evidence on the ethical challenges in implementing this plan.</span></span><span style="font-size:12.0pt"></span></span></span></span><br> <span style="font-size:8.5pt"><span style="line-height:17pt"><span new="" roman="" style="font-family:" times=""><b><i><span style="font-size:13.0pt">Materials and Methods:</span></i></b> <span style="font-size:12.0pt"><span style="letter-spacing:-.1pt">In this narrative review, an advanced search was conducted using a combination of Persian keywords equivalent to ‌&ldquo;Health Transformation Plan&rdquo;,‌&ldquo;Iran&rsquo;s health system&rdquo; ‌and ‌&ldquo;Ethical challenges&rdquo;,‌ along with their English counterparts ‌(&ldquo;Health Transformation Plan&rdquo;, &ldquo;Iran&#39;s health system&rdquo;, &ldquo;Ethical challenges&rdquo;) across international databases, including Web of Science Core Collection, PubMed/Medline, Scopus, the search engine Google Scholar, and national databases, such as SID, Magiran, and Irandoc, covering the time period from 2011‌ to‌ 2025.</span></span><span style="font-size:12.0pt"></span></span></span></span><br> <span style="font-size:8.5pt"><span style="line-height:17pt"><span new="" roman="" style="font-family:" times=""><b><i><span style="font-size:13.0pt">Results:</span></i></b>&nbsp; <span style="font-size:12.0pt"><span style="letter-spacing:-.1pt">The initial search yielded 880 articles. After removing duplicates and applying inclusion criteria, 17 ‌relevant qualitative studies focusing on the ethical principles in the implementation of the Health Transformation Plan&mdash;beneficence, non-maleficence, autonomy,‌ and justice&mdash;were analyzed.
