fa پیامدهای بهداشتی کارخانه سیمان دورود « تمرکز بر خطرسقط جنین در زنان باردار» : یک خلاصه سیاستی Health Impacts of Doroud Cement Factory :Focus on the Risk of Miscarriage in Pregnant Women: A Policy Brief زنان زنان <table align="right" class="Table" dir="rtl" style="width:596px; border-collapse:collapse; margin-left:10px; margin-right:10px" width="744"> <tbody> <tr> <td style="width: 744px; padding: 0in 7px; height: 1px; text-align: justify;" valign="top">مقدمه: سلامت باروری زنان از مهم‌ترین ارکان سلامت فردی و اجتماعی و یکی از شاخص‌های کلیدی توسعه انسانی است؛ به‌ویژه در جوامعی که با روند کاهشی نرخ زاد و ولد مواجه‌اند. در شهرستان دورود، کارخانه سیمان به‌عنوان یکی از صنایع بزرگ استان لرستان، علاوه بر نقش اقتصادی، پیامدهای نگران‌کننده‌ای بر سلامت جمعیت محلی دارد. زنان باردار، به‌عنوان گروهی آسیب‌پذیر، در معرض خطرات ناشی از آلودگی‌های زیست‌محیطی قرار دارند. شواهد علمی داخلی و بین‌المللی ارتباط معنادار میان تماس با ذرات معلق، گازهای سمی و فلزات سنگین با افزایش خطر سقط جنین و اختلالات بارداری را تأیید کرده‌اند. با این‌حال، ضعف در اجرای پیوست سلامت، نبود نظام پایش بهداشتی، فقدان سامانه دقیق ثبت سقط جنین، فقدان نظارت مستمر، خلاهای قانونی و پایین بودن آگاهی جامعه محلی، موجب تداوم این تهدید و پنهان ماندن بحران از دید سیاست‌گذاران شده است. این خلاصه سیاستی با هدف هشدار و ارائه راهکارهای مبتنی بر شواهد، وضعیت موجود را تحلیل کرده و عوامل اجتماعی، اقتصادی، فرهنگی و قانونی مؤثر در تداوم بحران را شناسایی کرده است. در بررسی شش گزینه سیاستی، «ارتقای سیستم کنترل آلودگی کارخانه» در کنار «مراقبت هدفمند از زنان باردار منطقه» به‌عنوان مداخله منتخب پیشنهاد می‌شود. اجرای این مداخله با کاهش خطرات کوتاه‌مدت و بلندمدت بارداری، افزایش اعتماد عمومی و ارتقای سلامت باروری همراه است. تحقق آن نیازمند اراده سیاسی، منابع مالی پایدار و همکاری بین‌بخشی میان شبکه بهداشت و درمان، سازمان محیط‌زیست، صنعت و معدن، فرمانداری و استانداری است. توجه فوری سیاست‌گذاران می‌تواند از گسترش آسیب‌های زیست‌محیطی و انسانی جلوگیری کرده و الگویی برای سایر صنایع آلاینده کشور فراهم آورد.</td> </tr> </tbody> </table> <div style="text-align: justify;">Background: Women’s reproductive health is not only a fundamental component of individual and social well-being but also a key indicator of human development and social sustainability. In recent decades—particularly in countries experiencing declining birth rates—its importance has significantly increased. In Dorud County, the cement factory, as one of the major industrial units in Lorestan Province, poses serious health concerns for the local population. Pregnant women, as a highly vulnerable group, are especially at risk due to continuous exposure to environmental pollutants.Extensive national and international evidence shows a significant association between exposure to particulate matter, toxic gases, and heavy metals with increased risks of miscarriage and adverse pregnancy outcomes. Despite these risks, weak implementation of health impact assessments, lack of a systematic health-monitoring program, absence of an accurate miscarriage registration system, inadequate environmental surveillance, legal gaps, and low public awareness have allowed this silent crisis to persist and remain largely unnoticed by policymakers.This policy brief aims to alert authorities and provide evidence-based solutions by analyzing the current situation and identifying the social, economic, cultural, and regulatory factors contributing to the crisis. Among six policy options evaluated, a combined intervention—“enhancement of the factory’s pollution-control system” and “targeted care for pregnant women in the affected area”—is recommended. This integrated approach addresses the pollution source while reducing both immediate and long-term pregnancy risks, improving public trust, and promoting reproductive health.Successful implementation requires political commitment, sustainable financial resources, and intersectoral collaboration between the local health network, the Department of Environment, the Industry and Mining Organization, and provincial authorities.
