Abstract

Background: Iodine deficiency and its related disorder such as goiter are endemic in Lorestan province. Following initiation of iodine deficiency control program in 1989, production, distribution and consumption of iodinated salt were begun. This survey was conducted in the framework of national monitoring survey in 2001 to find out the prevalence of goiter and urinary iodine level in order to evaluate the iodine status of school- aged children in Lorestan. Materials and Methods: 1200 schoolchildren, aged 7-10 years, were selected randomly from all regions of Lorestan. The grade of goiter in 600 boys and 600 girls, was determined according to WHO classification. Urinary iodine content was estimated using the digestion method in one tenth of the schoolchildren. Findings: Total goiter rate was 7.8% 7.8% in girls and 7.7% in boys. Median urinary iodine was 17 μg/dl. Urinary iodine was above 10 μg/dl in 85.7% and less than 5 μg/dl in 1.7%. No one had urinary iodine below 2 μg/dl. Conclusion: It is concluded that the rate of goiter in Lorestan has decreased significantly since 1996 and urinary iodine levels in schoolchildren are indicative of adequate iodine intake. Therefore Lorestan province can be considered as an 'iodine deficiency free' zone.

Abstract

Appendix carcinoid tumors are of rare cases. Their commonest place is in digestive system in appendix intensine and rectom respectively. They have no symptoms and they are detected along with laparotomy or inflation in appendix. The aim of the present study was to determine the frequency of carcinoid tumor in appendix and its prognosis in a period of 3 to 8 years in Shohada hospital in khorramabad. Materials and Methods: The present study is a cross – sectional which was carried out on 2845 patients with appendectomy in Shohada hospital in khorramabad in 2003-2006. The needed data were collected through a questionnaire from the pationts files who had the tumor operation in the hospital. The data were analyzed using descriptive statistics. Then the patients prognosis was determind. Results: The results showed that among all the cases with tumors, three out of five were carcdinoids, one was mucucell, one was mucucell associated lymphoid tissue. In adition to these cases, one case was colon carcinoid related to metastatic΄s deodenom, but no adenocarcinoma tumor was observed. After pathological analysis, the results showed that none of the patients with carcinoid appendix needed any treatment in 3-8 years follow up program and there was no recurrent growth for the disease. Conclusion: As carcinoid in appendix can be malignant and spread over other tissues and causes short life, it needs special care and follow and the patients should be referred to the specialist to complete treatment. Carcinoid appendix should be considerd as a neoplasm since it has the special power of spreading and metastasis over distant tissues.

Abstract

Trichomonas vaginalis is a flagellate protozoan that lives in the genital tract and causes trichomoniasis in women. About 200 million people all over the world are infected with T. vaginalis. There are various methods with different sensitivity and specificity for detection of this parasite, that one of them is direct smear of vaginal secretions which is simpler, rapid and cheaper than other diagnostic methods. Materials and Methods: Demographic data was gathered by a questionnaire which contained different variables. Vaginal secretions samples were taken by spicolum and two swaps that maintained in glucose solution in separate tubes from 160 women referred to health centers of Khorramabad. One of the vaginal samples was examined by direct smear in saline solution and the other was cultured in Dorse medium. Results: Of 160 women suspected of trichomoniasis, 11.8% and 18.75% were positive by direct smear and culture respectively. The sensitivity of the direct method was 63.3%. Our findings indicated that 30% of the infected women belonged to the 31 – 35 age group, which had the most relative frequency of positive cases. Most of the patients (43%) were illiterate or had elementary educational level. Conclussion: The sensivity of direct method is 63% in compare to culture ( as a Gold standard ), which is ralatively low . Although the efficacy of this test could be imporved by shortening the elapsed time between sampling and examination , use of skilled microscopists , and different samples , but we recommend that more sensitive methods such as culture and PCR should be used .

Abstract

Background: Pseudomonas aeruginosa is a gram-negative bacillus that an opportunistic pathogen in patients with immune system disorder is known. One of the common causes of nosocomial infections is considered. Organisms oxidase positive, catalase positive, animated, and aerobic and lacks the ability to ferment carbohydrates. Group B beta-lactamase, which called MBL. Since the range generally makes antibiotics such as penicillin, cephalosporin, carbapenem broad spectrum and (with the exception of monobactams) are effective, are the clinical problem. Metallo -beta-lactamase (VIM/ IMP) are plasmid genes. The aim of this study was to evaluate the phenotype and genotype of MBL genes VIM-I and IMP-I in clinical isolates of Pseudomonas aeruginosa imipenem resistant to antibiotics.
Materials and Methods:in this study by cultivating in public and differential culture and perform biochemical tests isolated for definitive confirmation and study metallobetalactamase genes from PCR was used.For review antibiotic resistance and study  metallobetalactamase genes phonotype by disc diffusion.
Results: Group study, Pseudomonas aeruginosa isolates were collected from Beheshti Hospital in Qom. Then MBL genes VIM-I and IMP-I after amplification with along 261 bp and 587 bp were observed on gel electrophoresis. From 100 isolates of P. aeruginosa examined 48 isolates (48%) MBLproducers.  Forty eight samples was MBL producers. 19 samples (58.39%) have bands of molecular weight of 587 gene IMP and 6 samples (5.12%) have bands of molecular weight of 261 gene VIM also 48 strains of 3 samples (25.6 percent) were containing both genes .antibiotic resistance to gentamicin samples, 36 samples (74%), ciprofloxacin 30 samples (62%), ceftazidime 29 samples (61%) ceftizoxime 27 samples (58%) and imipenem 19 samples (39%) were resistant. Conclusion: Compared IMP gene alone fisher test P≤0.05 significant relationship was found while the phenotype and gene VIM alone test, fisher P≤0.05 was used, there was no significant relationship. as a result of infection and region and type of antibiotics used in the sector can affect the expression of MBL.

Abstract

Background: Doxorubicin has been used in the treatment of malignancies, including lymphoma, leukemia and breast cancer. Cardiotoxicity is the main adverse effect of doxorubicin. Apigenin, as a flavonoid, has antioxidant, anti-inflammatory and anti tumoral properties. The aim of this study is the assessment of the effect of apigenin on cardiotoxicity induced by doxorubicin.
Materials and Methods: 60 male wistar rats were divided into 6 groups. Cardiotoxicity was induced by 6 injections of doxorubicin (2 mg/kg, ip) over 12 days. The treatment groups received orally 25, 50 and 75 mg/kg/day apigenin for 12 days simultaneously with cardiotoxicity induction.
Results: The heart weight to body weight ratio showed no significant difference between different groups. In the apigenin group (25 mg/kg), EF and FS showed  a significant increase (P<0.05) and LVEDs, LDH and CK-MB showed a significant decrease, in comparison to the cardiotoxicity group.
Conclusion: Apigenin prevents LDH and CK-MB elevation and also EF and FS reduction and improves cardiac tissue changes and leads to a decrease in cardiac damage induced by doxorubicin.

Abstract

Background: Patients with diabetes mellitus are at high risk of antioxidants and in long-term vascular lesions. Therefore, the aim of the present study was to evaluate the effect of one course of aerobic exercise on the gene expression of angiopoietin-1 (ANGPT-1) and cardiac tissue oxidative status (TOS) in diabetic rats.
Materials and Methods: 30 male Wistar rats were divided into 3 groups (n= 10) diabetic training, diabetic control and healthy control groups. Animals in diabetic groups were diabetic by intraperitoneal injection of streptozotocin. The exercise program consisted of 6 weeks of treadmill training. 24 hours after the last training session to evaluate the effect of exercise, cardiac tissue samples were extracted to measure gene expression  of ANGPT-1 and total oxidative status. One-way ANOVA with statistical level (P<0.05) was used to compare the differences between groups.
Results: The results showed that gene expression of ANGPT-1 and TOS were significantly increased in the diabetic training group compared to the diabetic control group (P= 0.001) but only in the ANGPT-1 there were no significant differences (P= 0.001) between two groups of diabetic control and healthy control (P>0.05).
Conclusion: It seems that endurance training has a positive effect on the expression of ANGPT-1 gene and improves the overall antioxidant status of the heart tissue of diabetic rats and improve the diabetic heart.

Abstract

Background: Menopause is a clinically significant period of time characterized by changes in ovarian hormone, vasomotor symptoms, and an increased risk of cardiovascular disease (CVD). One of the main causes of heart attacks is changes and imbalance of the homeostasis system. Sedentary lifestyle and aging also have adverse effects on the homeostasis system. One of the treatment methods that has been used for many years in the treatment of many diseases is Iranian-Islamic medicine. Therefore, in this study, the effect of cupping and aerobic exercise on LDL, HDL and fibrinogen was investigated
Materials and Methods: A total of 30 inactive postmenopausal women with an age range of 50 to 65 participated in this quasi-experimental study. Participants were randomly divided into three groups: aerobic exercise, cupping therapy, and control group. Aerobic exercises with an intensity of 50 to 75% of heart rate reserve were performed three times a week for 6 weeks and the cupping group was cupped in the third week. Blood samples were taken to measure fibrinogen, LDL, and HDL before the start of the training period and 48 hours after the end of the last training session. Then, an analysis of covariance was used for intergroup comparisons. Also, the pre- and post-test were analyzed using paired t-test at a significance level of <0.05.
Results: Aerobic exercise and cupping showed a significant decrease in fibrinogen (P = 0.04) and LDL (P = 0.001) and an increase in LDH (P = 0.003). Paired t-test results showed a significant decrease in fibrinogen, LDL and HDL post-test compared to pre-intervention post-test.
Conclusion: Cupping therapy and aerobic physical activity are effective factors in reducing the levels of fibrinogen and LDL and increasing HDL in postmenopausal women. According to the results, exercise may have better effects on decreasing fibrinogen and blood lipids, as the most important risk factors for cardiovascular disease and atherosclerosis, compared to cupping therapy.
