Background: The health system refers to a set of organizations, individuals, and actions aimed at improving community health, ensuring equitable participation in resource allocation, and enhancing patient satisfaction. Since‌ 2014, with the implementation of the‌ “Health Transformation Plan,” Iran’s health system has undergone significant reforms. One of the less explored aspects of this plan is its ethical challenges. The four core ethical principles—beneficence, non-maleficence, autonomy, and justice constitute the foundation of health policies. Therefore, the present study was conducted as a narrative review to examine the existing evidence on the ethical challenges in implementing this plan.
Materials and Methods: In this narrative review, an advanced search was conducted using a combination of Persian keywords equivalent to ‌“Health Transformation Plan”,‌“Iran’s health system” ‌and ‌“Ethical challenges”,‌ along with their English counterparts ‌(“Health Transformation Plan”, “Iran's health system”, “Ethical challenges”) across international databases, including Web of Science Core Collection, PubMed/Medline, Scopus, the search engine Google Scholar, and national databases, such as SID, Magiran, and Irandoc, covering the time period from 2011‌ to‌ 2025.
Results:  The initial search yielded 880 articles. After removing duplicates and applying inclusion criteria, 17 ‌relevant qualitative studies focusing on the ethical principles in the implementation of the Health Transformation Plan—beneficence, non-maleficence, autonomy,‌ and justice—were analyzed. The results indicated that adherence to these principles can significantly improve the quality of healthcare services and enhance the satisfaction of both healthcare personnel and the general public.
Conclusion: By paying attention to ethical dimensions in Iran’s health policies, strengthening education, optimizing resource allocation, and developing monitoring systems, service quality can be promoted and public trust increased.
