Background: Idiopathic short stature and its treatment pose major challenges in pediatric endocrinology. One of the treatments used is letrozole, and despite the high prevalence of idiopathic short stature, there is no specific treatment protocol for this condition. Therefore, this study aimed to investigate the effect of bone age at the start of letrozole treatment on the rate of height gain in boys with idiopathic short stature.
Materials and Methods: This retrospective cohort study was conducted in 1404 at two pediatric endocrinology centers in Tehran and reviewed the records of patients with idiopathic short stature. Sampling was census-based, and data from patients eligible for the study were extracted from their records and entered into Stata (version 17) for analysis.
Results: The optimal bone age breakpoint was determined to be 13.7 years. Before this point, each unit increase in bone age was associated with a 5.7-unit decrease in growth rate (β = -5.7). After this point, each unit increase in bone age was associated with a 7.38-unit decrease in growth rate, and the intervention lost its relative effectiveness (β = -7.38).
Conclusion: Letrozole is more effective for treating boys with idiopathic short stature when their bone age is less than 13.7 years. This can help in determining the appropriate treatment protocol and reducing the incidence of unwanted drug side effects.
