Background: Non-suicidal self-harm behaviors refer to direct and intentional behaviors of destroying body tissues without suicidal intent, which is currently considered one of the serious challenges for adolescent psychosocial health in the general population. The present study aimed to determine the effectiveness of emotion-focused schema therapy on cognitive flexibility and alexithymia in adolescents with non-suicidal self-harm behaviors.
Materials and Methods: The present study employed a quasi-experimental pretest-posttest design with a two-month follow-up and a control group. The statistical population comprised adolescent girls with self-injurious behaviors from Isfahan mental health service centers in 2024. Thirty participants were purposively selected based on the inclusion criteria and randomly assigned to two groups. The cognitive flexibility and alexithymia questionnaires were used to measure changes in the dependent variables. The experimental group received 9 sessions of emotion-focused schema therapy, administered once weekly. The data were analyzed using repeated-measures analysis of variance and a Bonferroni post hoc test.
Results: Emotion-focused schema therapy was effective in improving cognitive flexibility and reducing alexithymia (P<0.05), and these effects remained stable in the follow-up phase, particularly for the identifying feelings and oriented thinking components of the alexithymia variable.
Conclusion: Emotion-focused schema therapy was effective in improving cognitive flexibility and reducing alexithymia. It is suggested that mental health professionals in the field of adolescents should use this treatment as an effective intervention to manage and improve adolescents' self-injurious behaviors.
